Patient Rights & Responsibilities

Saint Alphonsus Medical Group adheres to the Core Values defined by Trinity Health: respect, social justice, compassion, care of the poor and underserved, and excellence.  Saint Alphonsus pledges to provide care that respects the dignity of each person and have outlined our commitment in “The Pledge of Patient Rights and Responsibilities.” A copy of these rights and responsibilities is posted in each clinic.

Medical Records

You have the right to obtain copies of your medical records. We will ask you to complete a form to give us permission to release you or your child’s records.  We require 7-10 days to process your request.

You may also access your records online through our NextMD Patient Portal. To establish a NextMD Patient Portal account, please visit your Saint Alphonsus Medical Group clinic. In person registration is required. If you have an existing NextMD account, you can access the patient portal at www.nextmd.com.
