Anticoagulation Clinic
The Saint Alphonsus Anticoagulation Clinic is a service established to help patients monitor and manage their warfarin therapy. Our clinical pharmacists and specially trained nurse are available to assist patients with their treatment. These providers check the patient's blood level via a fingerstick and adjust the warfarin dose as needed. In most cases, the patient will not require a venous lab draw, and will not need to wait for a phone call for dosing instructions. The referring provider receives the results via fax, and is consulted if there are any questions. Saint Alphonsus is proud to have offered this service to patients and physicians since 2005.
Using a patient-centered approach to warfarin therapy, our focus is to educate and empower patients on warfarin, to keep providers well-informed about their patient’s care, and to provide helpful, compassionate care to every patient.
Anticoagulation Services:
- Point-of-Care testing (through a finger-stick).
- Adjustment of warfarin dosing.
- Patient education.
- Screening for medication interactions/dietary changes.
- Assistance in bridging warfarin therapy around surgical procedures.
- Monitoring for side effects.
Patients must have a referral from a providerto schedule an appointment at one of our Anticoagulation Clinics. All visits are by appointment only.
