Saint Alphonsus encourages patient involvement in their health care by obtaining access to their personal health information. By viewing or accessing your records, you can be better informed about the treatment you receive and become an advocate for your own healthcare.

Saint Alphonsus uses a state-of-the-art electronic medical record system. Staying connected to your health has never been easier with convenient access to your records through our web-based patient portal, MyChart. It is secure and available 24/7 from your phone, tablet, or computer.

Request Copies of Medical Records

Most medical records are kept for a minimum of 11 years. Saint Alphonsus has two options to request copies of your medical records:

Option 1:

Online Request Application - FOR PATIENTS ONLY

Click the button below to submit a request through our secure online system with request status tracking. Please have your driver’s license or government issued ID ready to submit with your request. Records cannot be released without proper identification.

Option 2:

Download, complete, and submit a written authorization form.

Patients can order copies of their medical records by completing the Request for Access to Health Information form (linked below) and returning the completed form with a copy of your driver’s license or government issued ID to mailto:bo-him-releaseofinfo@saintalphonsus.org or SAHS HIM, 1055 N Curtis Rd, Boise, ID 83706. Records will not be released without proper identification.

 

 


Saint Alphonsus Medical Group

Contact each individual clinic directly for copies of medical records. Each clinic maintains their own medical records. Click to view a list of Saint Alphonsus Medical Group clinics.

Exception: In Baker City, all release of medical records for the Saint Alphonsus Medical Group clinics are handled by the Saint Alphonsus Medical Center - Baker City HIM department. See above for contact information.

Fees for Records

Copies for patients or their representatives may have a reasonable, cost-based fee. Please contact the Health Information Management Department at the corresponding phone numbers above for further information.

How soon can I expect to receive my requested medical records?

Most requests for medical records will be processed within 30 calendar days from receiving the completed request unless we notify you that the request cannot be processed within that time frame, the reason for the delay, and the date by which we will complete action on the request.
